HOW TO APPLY

Individual Leadership Development Program

Applicants for the Individual Leadership Development Program should prepare a 2 to 3 page proposal that provides the following information:

  1. The organization’s name along with the name, title and contact information of the key individual who will be participating in the program.
  2. A description of the organization, its mission, size, management structure and the services provided in Clay County.
  3. The name of, or a description of, the leadership training opportunity that the organization wishes to pursue, including the name of the provider, the time frame and the cost.
  4. Why and how the selected training program meets the needs of the organization.

With the proposal, please submit the following:

  1. A letter from the organization’s board chair supporting participation in the selected leadership training program.
  2. A current resume of the individual selected by the organization to participate in the training.
  3. A copy of the training program curriculum. (If using a consultant, please include a copy of the consultant’s credentials and client list.)
  4. A detailed budget for training, including tuition, travel, and other associated costs.
  5. A copy of the organization’s current operating budget and its most recent audited financial statements or 990 Tax Return. If your organization is not required to file a tax return, please state so and provide a copy of any existing financial statements.
  6. A copy of the most recent letter of exemption from the Internal Revenue Service demonstrating that the organization is exempt from federal income tax under Section 501(c )(3) of the Internal Revenue Code and classifying the applicant as “not a private foundation” under Section 509(a), (1) or (2) of the Internal Revenue Code. Also include a statement that the organization’s tax exemption under Section 501(c)(3) of the Internal Revenue Code and the status under Code Section 509(a), (1) or (2) have not been revoked or modified.

DEADLINE for applications:

Applications must be received by the close of business on Friday, November 9, 2007.

Awards:

Successful applicants will be notified by Wednesday, December 19, 2007.

Reporting Requirements:

Successful applicants will be required to submit an interim report in April 2008 and a final report upon completion of the program, or no later than September 15, 2008. The Paul E. and Klare N. Reinhold Foundation, Inc. may request a follow-up interview with the grantee 6-12 months after conclusion of the program.


Nonprofit Management Team Development Program

Applicants to the Nonprofit Management Team Development Program should prepare a 2 to 3 page proposal that provides the following information:

  1. The names of the team members (no more than four), their titles, their roles within the organization, the length of time they have been involved with the organization, and their contact information.
  2. A description of the organization, its mission, size, organizational structure and the services that the organization provides in Clay County.

With your proposal, please submit the following:

  1. A letter from the organization’s board chair supporting the organization’s application to the program.
  2. A copy of your organization’s current operating budget and its most recent audited financial statements or 990 Tax Return. If your organization is not required to file a tax return, please state so and provide a copy of any existing financial statements.
  3.
